Résumé

This thesis deals with the study of a mathematical model of growing metastatic tumors and its possible clinical applications. In particular, it is developed a theoretical and numerical study of the models. The first part of this manuscript is devoted to the study of a model of non treated metastatic growth where it is introduced a new in silico tool called Metastatic Index which could permit to quantify the metastatic staging of a given patient and could consequently improve the existing classifications of tumoral aggressiveness. The second part of this thesis deals with the study of the modified model in order to take into account a chemotherapy treatment. It is presented a theoretical and numerical study and some simulation examples aiming at studying the evolution of the metastatic index inflected by the action of the treatment.\newline The last part of this manuscript is devoted to study the potentialities of the model concerning clinical applications. in particular it is described a way to establish in some manner the reliability of the model comparing some results of simulations with real clinical results. Finally, it is presented a possible way aiming at refining the classification of the metastatic staging of a patient and how to determine the more efficient treatments in order to minimize the occurrence of recurrences.
Cette thèse traite de l'étude d'un modèle mathématique de croissance tumorale métastatique et de ses éventuelles applications en clinique. Il y est développé en particulier une étude théorique et numérique des modèles présentés. La première partie du manuscrit traite de l'étude d'un modèle de croissance métastatique non traitée où il est introduit un nouvel outil in silico: l'Index Métastatique qui pourrait permettre de quantifier l'état métastatique métastatique d'un patient donné et qui pourrait ainsi venir renforcer les classifications tumorales déjà existantes. La deuxième partie traite de l'étude du modèle enrichi de manière à prendre en compte l'action d'un traitement par chimiothérapie. Il y est présenté une étude théorique et numérique du modèle modifié ainsi que des exemples de simulation visant à étudier l'évolution de l'index métastatique infléchi par l'action du traitement. La dernière partie de ce manuscrit est consacrée à l'étude des potentialités du modèle en termes d'applications cliniques. Il y est décrit en particulier une manière de rendre compte de la fiabilité du modèle en confrontant les simulations avec des résultats provenant d'une étude clinique. Enfin il est étudié quelques pistes pour raffiner la classification de l'état de la maladie d'un patient et de comment déterminer les traitements les plus efficaces afin de minimiser la survenue de récidives.
