Résumé

Osteoarthritis, asymptomatic disease until reaching an advanced stage, represent a major health problem, the most efficient treatment being the replacement of the joint by an implant. Despite numerous optimization studies, the in vivo lifespan of implants remains very disappointing compared to extrapolations from ex vivo simulations. In this context, the purpose of this study is to identify the wear mechanisms of the knee implants (third body flows, velocity accommodation mechanism of CrCo / UHMWPE bearing surfaces) and to understand their in vivo tribological behaviour compared to the ex vivo simulations, as a response to optimize their manufacturing and to increase their in vivo lifespan. The first results from in vivo tribological investigation of two retrieved implants revealed two different wear mechanisms leading to wear rates of 1 :10. Low wear rates have been correlated with the presence of a third body layer, constituted by a mixture of large particles of UHMWPE and synovial fluid "trapped inside the contact". The high wear rates have been correlated with the ejection of small particles outside the contact. To explain the origin of these flows, two tribometers have been adapted: first one, dedicated to reproduce more precisely the knee implant surfaces kinematics and the second one to analyze in situ the different activated flows. Moreover, a biomimetic synovial fluid has been developed by reproducing the composition and the structure of the real synovial fluid. The tribological behavior of this synovial fluid has been compared with the standard lubricant according to the ISO14243-1 standard. Measurements of the friction coefficient correlated with the in situ visualization of the third bodies flows allowed the reconstruction of the in vivo friction mechanisms. This reconstruction revealed the role of two optimization parameters: one related to the machining of the UHMWPE surface, beeing able to trap the lubricant and promoting the most hydrodynamic bio-lubrication possible; the other linked to the increase of bearing capacity of the third body by phopholipid vesicles.
L’arthrose, maladie asymptomatique jusqu’à un stade avancé, pose un problème de santé majeur, le seul traitement efficace est la pose d'un implant; ce qui n’est qu’un pis-aller car malgré de nombreuses études d’optimisation, la durée de vie in vivo de ces implants reste encore très décevante par rapport aux extrapolations issues de simulations ex vivo. Dans ce contexte, l’objectif de cette étude est d’identifier les mécanismes d’usure des implants de genou (débits source de troisième corps, couple de frottement CrCo/UHMWPE) et de comprendre leur fonctionnement in vivo par rapport aux simulations ex vivo afin de trouver des solutions pour optimiser leur fabrication. Les premiers résultats issus d’expertises tribologiques in vivo ont révélé deux mécanismes d’usure conduisant à des débits d’usure variant dans un rapport dix. Les faibles débits d’usure ont été corrélés avec la présence d’une couche de troisième corps, composée par un mélange de grosses particules d’UHMWPE et de liquide synovial qui «restent en contact». Les forts débits d’usure ont été corrélés au débit d’éjection de petites particules. Pour expliquer l’origine de ces débits, deux tribomètres ont été adaptés: l’un pour reproduire plus précisément les mouvements du genou et l’autre pour analyser in situ les différents débits activés. Pour cela, un lubrifiant biomimétique a dû être mis au point en reproduisant la structure et la composition du liquide synovial. Le comportement de ce liquide a été validé par comparaison avec celui du lubrifiant standard ISO14243-1. Des mesures du coefficient de frottement corrélées avec le suivi in situ des débits de troisièmes corps ont permis de reconstituer les mécanismes de frottement in vivo. Cette reconstitution a révélé le rôle de deux paramètres d’optimisation: l’un lié à l’usinage de la surface en UHMWPE pour qu’elle piège du lubrifiant tout en favorisant une bio-lubrification la plus hydrodynamique possible; l’autre lié à l’accroissement de la portance du troisième corps par des vésicules lipidiques.
