Contribution to the analysis and modeling of the spatio-temporal dynamics of the incidence rate of arboviruses in Djibouti

Résumé
As part of this thesis, we present an epidemiological study on the evolution fevers of unknown origin in Djibouti over a period of 10 years. Indeed, fevers of unknown origins are defined by a body temperature above 38 degrees which is not associated with any transient or selflimiting illness. They are part of the arboviruses which are diseases transmitted by bites of haematophagous arthropods such as ticks, fleas or mosquitoes. Thus, in the epidemiological study, a suitable tool is the mapping of risk which makes it possible to represent the spatiotemporal variations of the risk of occurrence of a pathology. Concretely, this consists of a visualization of the data by a map using of a health indicator and in our case, the SIR (Standardized Incidence Ratio) is chosen. Throughout this thesis, we make estimates of the SIR by combining it, on the one hand, with socio-demographic data and, on the other, the models considered in our case which are described as follows : • The Poisson model: In order to model the risk and the risk factors independently the geographical position then make a diagnosis on the need for a spatiotemporal study,• Bayesian hierarchical models: In order to produce smooth SIR maps allowing to highlight possible smooth spatial and temporal variations (gradients) and / or spatio-temporal of the SIR taking into account or not the cofactors and these cofactors which may be meteorological factors. In particular, the CAR models (Conditional AutoRegressive) spatial, temporal and / or spatio-temporal: To integrate a smoothing of the neighborhood system by modeling the evolution of the SIR according to dissimilarities local (by neighborhood) or group together those with the same temporal growth.It is common in the literature to have maps produced from an optimal choice of CAR model according to a given criterion. Assuming that each CAR model can provide interesting information, we have chosen to make a compromise between model selections classic and search for additional information in the map produced by each model.
Nous proposons, dans cette thèse, des méthodes de cartographie du risque dans le cadre d’une étude épidémiologique sur l’évolution des fièvres d’origines inconnues à Djibouti sur une période de 10 années. En effet, pour le ministère de la santé de Djibouti, les fièvres d’origines inconnues sont définies par une température corporelle mesurée supérieure ou égale à 38.3 degrés qui ne s’associe à aucune maladie identifiée, qu’elle soit transitoire, autolimitée ou d’une durée supérieure à une semaine. A Djibouti, elles sont le plus souvent décrites a posteriori comme étant corollaires des arboviroses et plus généralement des maladies vectorielles transmises par piqures d’arthropodes. Ainsi, dans l’étude épidémiologique, un outil adapté est la cartographie de risque qui permet de représenter les variations spatio-temporelles du risque d’apparition d’une pathologie. Concrètement, cela consiste en une visualisation des données par une carte à l’aide d’un indicateur sanitaire et dans notre cas, le SIR (Ratio d’Incidence Standardisé ou Standardized Incidence Ratio en anglais) est choisi. Tout au long de cette thèse, nous réalisons des estimations du SIR en le combinant, d’une part, avec des données sociodémographiques puis de l’autre, les modèles envisagés dans notre cas qui sont décrits comme suit :• Le modèle de Poisson : Afin de modéliser le risque et les facteurs de risque indépendamment de la position géographique puis faire un diagnostic sur la nécessité d’une étude spatiotemporelle,• Les modèles hiérarchiques bayésiens : Dans le but de produire des cartes lisses du SIR permettant de mettre en évidence d’éventuelles variations (gradients) lisses spatiaux, temporels et/ou spatio-temporels du SIR en tenant compte ou non des cofacteurs et ces cofacteurs pouvant être des facteurs météorologiques. Particulièrement, les modèles CAR (Conditional AutoRegressif) spatiaux, temporels et/ou spatio-temporels : Pour intégrer un lissage du système de voisinage à travers la modélisation de l’évolution du SIR selon les dissimilarités locales (par quartiers) ou encore regrouper ceux ayant la même croissance temporelle.Il est courant dans la littérature d’avoir des cartes produites à partir d’un choix optimal de modèle CAR selon un critère donné. Partant du principe que chaque modèle CAR peut apporter une information intéressante, nous avons choisi de faire un compromis entre sélections de modèle classique et recherche d’informations complémentaires dans la carte produite par chaque modèle.
