Résumé

Paediatric obesity, which has been recognised as one of the world's major new diseases since the early 2000s from both a medical and sociological perspective, is struggling to be controlled. Faced with this problem, the Ministry of Social Solidarity and Health has reacted by proposing measures aimed at adopting "good practices" (PNNS 1 to 4). At the same time, it has introduced the development of new structures (health networks) to support the most complex cases with the help of a multidisciplinary team. Organised according to the model of therapeutic patient education (TPE), the latter aims to make patients autonomous in the day-to-day management of their illness, which represents a major paradigm shift in the world of health. In parallel with this health issue, the digital world has seen an exponential increase in technological innovations such as computers, smartphones and tablets over the last few years. These have increased the rate of sedentary behaviour among children and adolescents, which has become one of the cornerstones of overweight and paediatric obesity. In view of these observations, combined with field observations, this research-action, which began in 2017, made it possible to approach our problem from a holistic point of view, taking into account the child's environment, societal changes and new epistemologies of care. A "parent-child" effort training programme was therefore proposed to different groups by integrating a digital innovation and specific sequences according to precise modalities. The implementation of this programme and the analysis of our theoretical tools enabled us to evaluate the potential benefits of mobilising a digital innovation in the support of paediatric obesity and to understand its appropriation scheme from the point of view of both teaching and the environment. After analysis and cross-referencing of the results, the first result is that the environment and the way it views the digital innovation is an important factor to take into account in the appropriation process. But also, if the new generations seem to master the digital tools with ease, it appears that specific pedagogical support must be offered in order to tame the tool. Finally, it would seem that integrating a digital innovation into a TVE cycle would enable participants to be more active on a daily basis and thus to have a better state of health in the case of a complete appropriation process.
L’obésité pédiatrique, reconnue comme un des nouveaux maux majeurs à l’échelle mondiale depuis le début des années 2000 aussi bien sur le versant médical que sociologique, peine à être contrôlée. Face à cette problématique, le ministère des solidarités sociales et de la santé a réagi en proposant des mesures visant à adopter de « bonnes pratiques » (PNNS 1 à 4). Parallèlement, il a instauré le développement de nouvelles structures (réseaux de santé) permettant d’accompagner les cas les plus complexes à l’aide d’une équipe pluridisciplinaire. Organisées selon le modèle de l’éducation thérapeutique du patient (ETP), ces dernières visent à le rendre autonome dans la gestion de sa maladie au quotidien, ce qui représente un changement fort de paradigme dans le monde de la santé. Parallèlement à cette problématique de santé, le monde du numérique, a connu depuis quelques années une augmentation exponentielle des innovations technologiques telles que les ordinateurs, les smartphones ou encore les tablettes. Ces dernières ont renforcé le taux de sédentarité chez les enfants et adolescents, devenu une des pierres angulaires du surpoids et de l’obésité pédiatrique. Au regard de ces constats, croisés aux observations de terrain, cette recherche-action débutée en 2017 a permis d’aborder notre problématique selon un prisme holistique tenant compte à la fois de l’environnement de l’enfant, des mouvances sociétales et des nouvelles épistémologies de prise en charge. Un programme de réentrainement à l’effort « parent-enfant » fut donc proposé à différents groupes en y intégrant une innovation numérique et des séquences spécifiques selon des modalités précises. La réalisation de ce programme et l’analyse de nos outils théoriques nous ont permis d’évaluer le potentiel bénéfice de mobiliser une innovation numérique dans un accompagnement de l’obésité pédiatrique et d’en comprendre son schéma d’appropriation aussi bien du point de vue de l’enseignement que de l’environnement. Après analyse et croisement des résultats, il en résulte tout d’abord que l’environnement et le regard qu’il porte sur l’innovation numérique est un facteur important à prendre en compte dans le procès d’appropriation. Mais encore, si les nouvelles générations semblent maîtriser les outils numériques avec aisance, il en ressort qu’un accompagnement pédagogique spécifique se doit d’être proposé afin d’apprivoiser l’outil. Enfin il semblerait que le fait d’intégrer une innovation numérique dans un cycle d’ETP permettrait aux participants d’être plus actifs au quotidien et ainsi d’arborer un meilleur état de santé dans le cas d’un processus d’appropriation complet.
