Résumé

The processes of change in addiction result from the interaction of an individual's characteristics (personality traits, genetic profile, cognitive characteristics...) with the environment (social, cultural). The objective of this thesis was to 1) characterize in populations suffering from severe alcohol use disorder (AUD), the early changes in cognitive disorders after cessation of alcohol consumption 2) the impact of certain somatic comorbidities, in particular cirrhosis, on alcohol-related cognitive anomalies. In a second approach we have 3) evaluated the impact of environmental changes during the Covid-19 epidemic on alcohol consumption in a population of Internet forum users. In a first study, 32 severe alcohol-dependent patients without psychiatric, neurological, or somatic comorbidities were evaluated using the Brief evaluation of alcohol related neuropsychological impairement (BEARNI), 8 days (T1) and 21 days (T2) after stopping alcohol consumption. The evolution of cognitive scores on the BEARNI (memory difference, fluency, working memory, visual-spatial abilities) was evaluated, as well as the proportion of subjects with impaired cognitive functions at T1 who recovered at T2. In a second study, the mean BEARNI scores of 75 patients with AUD and cirrhosis were compared to the mean scores of 32 patients with AUD without cirrhosis, considering the severity of liver damage and the level of abstinence in the analysis of the results. In a third study, we evaluated changes in alcohol consumption in a sample of 974 subjects using an online survey during the containment of the Covid-19 epidemic. The results of this thesis work found that 75% of participants in a group of subjects with severe AUD showed a normalisation of BEARNI scores after an average of 21 days of abstinence. The second study found that patients with AUD with cirrhosis (n=75) had significantly lower mean BEARNI cognitive scores than patients with AUD without cirrhosis (n=32). The third study found that 74% of the subjects in a group of drug users' forums had changed their alcohol consumption, of which 41.6 increased, 22.4 % decreased and 10% stopped their alcohol consumption.Change in addiction can occur at the genesis and in the consequences of addictions. Change can lead to or contribute to the initiation or worsening of an addictive disorder or to an improvement of the addictive disorder. Among the factors related to the individual, cognitive alterations related to alcohol consumption have a marked influence on decision-making, adherence to care and long-term evolution. Our contribution has clarified some of the changes in cognitive processes during remission from alcohol dependence and some of the changes in alcohol consumption during the changes induced by the Covid-19 epidemic. Understanding these dynamics of change in the determinants and processes of remission in addiction has led to further studies of emerging nitrous oxide and cannabidiol use. As consumption patterns are changing very rapidly, monitoring trends and adapting to a changing environment is an important issue in Addictology
Les processus de changement dans les addictions résultent de l'interaction des caractéristiques d'un individu (traits de personnalité, profil génétique, caractéristiques cognitives…) avec l'environnement (social, culturel). L'objectif de ce travail de thèse était de 1) caractériser dans des populations souffrant d'un trouble de l'usage d'alcool (TUA) sévère, les changements précoces des troubles cognitifs après l'arrêt des consommations d'alcool 2) l'impact des certaines comorbidités somatiques notamment la cirrhose sur les anomalies cognitives liées à l’alcool. Dans une deuxième approche nous avons 3) évalué l'impact de changement de l'environnement lors de l'épidémie de Covid-19 sur les consommations d'alcool dans une population d'usagers de forums Internet. Dans un premier travail, 32 patients alcoolodépendants sévères sans comorbidités psychiatriques, neurologiques ou somatiques, ont été évalué à l’aide de la Brief evaluation of alcohol related neuropsychological impairement (BEARNI), 8 jours (T1) et 21 jours (T2) après l'arrêt des consommations d'alcool. L'évolution des scores cognitifs au BEARNI (mémoire différence, fluence, mémoire de travail, capacités visuo-spatiales) a été évaluée, ainsi que la proportion de sujets avec des fonctions cognitives altérées à T1 ayant récupéré à T2. Dans une deuxième étude, les scores moyens au BEARNI de 75 patients avec un TUA et souffrant de cirrhose ont été comparés aux scores moyens de 32 patients souffrant de TUA sans cirrhose, en prenant en compte la sévérité de l'atteinte hépatique et le niveau d'abstinence dans l'analyse des résultats. Dans une troisième étude, nous avons évalué les modifications de consommation d'alcool d'un échantillon de 974 sujets à l'aide d'une enquête en ligne durant le confinement lié à l'épidémie du Covid-19. Les résultats de ce travail de thèse ont retrouvé que 75 % de participants d'un groupe de sujets présentant un TUA sévère présentait une normalisation des scores au BEARNI après 21 jours d'abstinence en moyenne. La deuxième étude a retrouvé que les patients souffrant de TUA avec cirrhose (n=75) présentaient des scores cognitifs moyens évalués au BEARNI, significativement inférieurs aux patients avec un TUA sans cirrhose (n=32). La troisième étude a retrouvé que 74% des sujets d'un groupe d'usagers de forums de consommateurs de drogues avaient modifié leur consommation d'alcool, dont 41.6 ont augmenté, 22.4 % ont diminué et 10% ont arrêté leur consommation d'alcool. Les changements en addictologie peuvent se rencontrer à la genèse et dans les conséquences des addictions. Le changement peut aboutir ou contribuer à l'initiation ou l'aggravation d'un trouble addictologique ou à une amélioration du trouble addictologique. Parmi les facteurs liés à l'individu, les altérations cognitives liées à la consommation d'alcool ont une influence marquée sur la prise de décision, l'adhésion aux soins et l'évolution à long terme. Notre contribution a permis de préciser certains changements à l'œuvre dans les processus cognitifs lors de la rémission de l'alcoolodépendance et certains changements dans la consommation d'alcool lors des changements induits par l'épidémie de Covid-19. Comprendre ces dynamiques de changements dans les déterminants et les processus de rémission en addictologie a permis d'engager d'autres études sur la consommation émergente de protoxyde d'azote et de cannabidiol. Les modes de consommation évoluant très rapidement, suivre les tendances et s'adapter à un environnement qui change est un enjeu important de l'Addictologie
